Pharmacology

Amlodipine is a calcium channel blocker which was originally marketed under the name Norvasc brand name since 1996.

Amlodipine was shown to inhibit the entry of calcium ions into the vascular smooth muscle and slightly increase the excitability of sodium and chloride absorption sites. Amlodipine is a sympathomimetic that acts to modify the chemical substance released during bronchodilators. This prevents the entry of calcium ions into the vascular smooth muscle.

Amlodipine has no direct vasodilator, amlodipine is a partial activator. It is a calcium-channel blocker which is thought to act by opening the channels for calcium influx. This action enables the smooth muscles of the blood vessels to dilate, which allows the vessels to fill with calcium when they are not dilating. This allows the vessels to empty again.

Amlodipine is a calcium channel blocker which acts on the presystemic alpha-adrenergic receptors. It is believed to be at least partially atvially iontophoretic. It acts at the presystemic beta-type receptors which are thought to be the receptors for the reuptake of calcium. It also acts on the sympathetic (antiandrogens such as Norvasc) and dopamine type 1 receptors which are thought to be the receptors for dopamine.

It has no direct action on the calcium reuptake at the entry site of angiotensin converting enzyme (ACE) in the heart. It only has an effect at the sites of ischemia and is not thought to cause any significant morbidity or mortality. Amlodipine does not have a direct effect on other proteins. It does not affect the function of vascular endothelial cells.

Amlodipine has been shown to be effective in controlling blood pressure in asymptomatic hypertensive patients with angina and in the control of blood pressure in asymptomatic hypertensive patients with normal hemophilic organ function.

Amlodipine has not been shown to cause a clinically significant increase in blood pressure. It has been used in the treatment of angina in combination with a diuretic to reduce suprapubanic sudsional pressure.

The safety and efficacy of amlodipine in the treatment of angina has not been established.

Amlodipine is not indicated for use in the management of coronary artery disease.

Dosage.Initial- or maintenance-dose:125 mg to 300 mg initially. Continue to take amlodipine at a dose of 125 mg to 300 mg on alternate days, at approximately 2-4 times the standard dose. Continue to take amlodipine for at least 7 days (depending on total daily dose). Continue to take amlodipine for symptomatic relief of mild to moderate heart failure.

Dose Adjustment:

The daily dose of amlodipine may be increased to a maximum of 400 mg to 600 mg initially, at a daily dose of 400 to 600 mg during the second week of treatment. The dose may be decreased to a maximum of 25 mg/day in the third week. The maximum recommended dose of amlodipine is 50 mg per day.

If the amlodipine is taken by or on demand, the dose should be stopped and the dose of amlodipine taken immediately before breakfast.

Use Caution:

Do not use the dose without consulting your doctor as a starting point. Do not increase the dose of amlodipine greater than or equal to the maximum recommended dose.
